Using the NFHS Medical Insurance Coverage

If you get injured while you are officiating these are the procedures to use the NFHS Officials Association Medical Insurance.

In a nutshell, the NFHS Medical Insurance is a supplemental insurance policy if you carry primary medical insurance from your own job. If you don’t have any insurance, the NFHS Medical Insurance will become your primary insurance (subject to the deductibles and limits of the plan).

First, for your records, get an official from the Host school to get you a letter on the school letterhead explaining that you were officiating for them and were injured during the game or match.

Depending on if you have insurance or not determines your next step:

• (with primary insurance) - See your doctor for your injury. Pay your co-pay for the visit and wait for the Explanation of Benefits (EOB) to arrive in the mail.
• (without primary insurance) - See your doctor for your injury. Pay the bill and get a copy of the bill.

Third, get a claim form by following these steps:

• Go to the NFHS website (http://www.nfhs.org)
• Look under the heading “NFHS FOR…” and click on the “Officials” link
• At the bottom of the next page, click on the “Officials Association Membership Benefits” link
• Two thirds of the way down the next page clock on the “Insurance Coverage” link
• On the next page, in the center, click on the “NFHS – Officials Association” link. This will take you to the information about the Insurance Summary for the NFHS Officials Association.
• Read the eligibility requirements
• Near the bottom of the page in a box labeled “Plan Administrator” are the contact numbers for Dissinger Reed (the insurance company)
• Call or email to request a claim form

You will need to submit this Claim form along with copies of the EOB’s (Explanation of Benefits) from your primary insurance carrier or copies of your doctor’s bills for reimbursement.
